To reactivate your account, complete a TAB Revocation and Re-Activation Application Form:

Ensure you complete the correct form for your state or territory and provide the supporting documentation under section 2 of the form. Email or post your application and all supporting documents to or PO Box 248 Albion QLD 4010.
